Insurance & Financing
The fees charged are based on the time and skill required for each procedure. Because processing bills is expensive, we request payment at the time service is rendered unless previous arrangements have been made. This helps us keep our fees as low as possible.
We are participating providers for numerous insurance companies, as well as Medicare. If we are participating providers with your insurance and your diagnosis is of a medical nature, we will file the insurance for you and accept assignment for all services rendered.
You will be responsible for routine examination charges, any deductible not met, and co-insurance percentage not paid on the approved amount. You may want to verify with your insurance that we are a participating provider prior to your visit.
If we are not listed as a provider with your insurance company, we will be happy to furnish you with an itemized statement which you can file with your insurance company for reimbursement. Please be sure to provide us with complete insurance information when you check in with the receptionist.
A nominal charge will apply for the completion of Disability and Proof of Loss insurance claim forms. If you have any questions about insurance or billing, please call us and speak with one of our Business Office personnel.
In order to make vision correction as affordable as possible, we accept a variety of payment options, including:
- cash
- personal checks
- cashier's checks
- credit cards (Visa, MasterCard, Discover, American Express)
- flexible spending accounts
- financing through CareCredit and ChaseHealthAdvantage.
Flexible Spending Accounts
Flexible Spending Accounts, also known as Medical Spending Accounts, can be used to pay for medical procedures, including refractive surgery. With a Flexible Spending Account, money is deducted from your salary pre-taxed, and you can decide how much you want deducted from each paycheck. Because the money is not taxed, you can save as much as 40% on your vision correction procedure by using a Flexible Spending Account!
